Jack Lonshein (1926-2015) was a noted designer and illustrator of LP jackets that included many jazz musicians of the era. This book has a discography of his work and includes many photos on glossy stock of his work. There is no indication of the publisher, but it was "Distributed by Funkadelic Record Distribution of Philadelphia, PA" and was probably published by the author who spent a number of years on the project.
