Cook" for "medical services for self" from September 16th, 1825 until April 21st, 1829, totaling $14.42, and noting already received payment for medical services rendered to Gerry Blakeslee for $9.00. Located at Plymouth. Manuscript fragment measuring approx. 5.5" by 7.75
