Includes a tipped-in photograph of Miller by Ina Paulandre, a self portrait by Hans Reichel, and textual drawings and reproductions. Square octavo. Original stiff white cardstock boards, printed in blue on the front panel. The dedication page is bound in upside down. A fine copy in a fine dust jacket, housed in the publisher's slipcase.
